Residentís from the City of Burlington, Towns of Burlington, Dover, Norway, Raymond and Yorkville Villages of Rochester and Waterford Per Person Administration Fees :

   1 immunization = $10        2  immunizations = $15         3 or more immunizations = $20


Non-Residentís Per Person Administration Fees: 

      1 immunization = $20         2  immunizations = $25         3 or more immunizations = $30   

Badger Care/Medicaid: Fee Exempt With Card

As of October 1, 2012, Section 317 of the Affordable Health Care Act states that health departmentís will only be able to provide immunizations to children through 18 years of age who are:  Medicaid eligible, American Indian, Alaska native, individuals with no health insurance, or the underinsured (has health insurance, but the coverage does not include all vaccines or only covers selected vaccines). 


Childrenís vaccines available for administration fee only:  Chicken Pox,  Diphtheria, Hepatitis A, Hepatitis B, Hib Meningitis, HPV, Measles, Meningococcal, Mumps, Pertussis, Pneumococcal, Polio, Rotavirus, Rubella, Tetanus and Influenza (in season).


Adultís vaccines available for administration fee:

        Tdap (Tetanus, Diphtheria, Pertussis also called whooping cough)

        TB Skin Tests are also available ($10 plus administration fee, must call 24 hours in advance to schedule).
