Reporting

The following diseases must be reported to the Health Department by state statute. Please REPORT IMMEDIATELY ANY OF THE FOLLOWING DISEASES whether verified by a physician or just suspected. The following information is requested when reporting the disease: name of individual name of parents/legal guardian if under 18 years of age home address home phone number date of birth date of onset  Public Health Nurses do an epidemiological follow-up. Education will also be provided on mode of transmission and prevention.
