Reporting

The following diseases must be reported to the Health Department by state statute. Please REPORT IMMEDIATELY ANY OF THE FOLLOWING DISEASES whether verified by a physician or just suspected.

The following information is requested when reporting the disease:

  • name of individual

  • name of parents/legal guardian if under 18 years of age

  • home address

  • home phone number

  • vaccination history

  • date of birth

  • date of onset
